Personal Health Records (PHR)

 

 

Health information is available in our personal health records (PHR) [ at www.healthvault.com and elsewhere.]

 

1. Check our wallets [and mobile devices] for basic emergency health information [and the access codes to our PHR on HealthVault].
    

2. Check keychains for a USB thumb drive. If one exists, it will contain electronic copies of more detailed emergency health information, similar to that accessible on HealthVault. Plug the thumb drive into a desktop computer, laptop, or mobile device with a USB port to access the information.

 

 

Advance Healthcare Directives (AHD)

 

Copies of Advance Healthcare Directives are available:
