Pet Advance Directive:

 

The Pet Advance Directive (PAD) will be included,here, under this heading, and copies of the PAD will be given to [name].

 

Interim Care:

 

If there is no one to take care of our pets daily needs for an anticipated interim period of time, they should be taken to [name, location] for temporary boarding.

 

Finding a New Home for Pet:
